how to get values from PDF form

I should write a JSP application that uses PDF files as a form to collect data from users. How can I read values from the form? I mean PDF, no XFT or anything else.
Or ... is there a way to convert PDF back to XSL-FO or any readable format?

Have you considered Scriptura?  http://www.inventivedesigners.com/products/scriptura.html

It may be better than attempting to write this kind of thing yourself ... it isn't trivial.
