Please verify that there are no Yahoo or Google Pop Blockers. I have seen this happen before because of that

Alternatively you could change the value of ‘Launch in app mode' to 'OFF’ in the web.config file which is located at the root of the wwwroot directory at the IIS.

This option determines whether or not MS CRM should be loaded in a browser window that does not have address, tool and menu bars. The default setting is “ON” only showing the application and not the browser, however this causes the application to be regarded as a popup and gets blocked by the blocking features of the Yahoo and Google bars.

Turning the value to “OFF” will show a browser window around the application, but allows it to work fine with the Google bar.

In case you are wondering why must you disable this tool? Because Microsoft CRM uses the Window.open command in the Default.aspx file, and pop-up blockers effectively block this command.

I don't know if you are still having this problem but I was having the same issue when I would login in remotely.  I solved it by typing http://Servername.com/loader.aspx  instead of http://Servername.com and the username and password box will display and then gives access.
