Couple of questions

a) What version of outlook you have problem in ?

b) What operating system ?

c) Does this user alone got an issue ?

Looks to me there could be several reasons for it . One such example is here

OL2000: Failure When Opening a Form on Windows NT 4.0 and WINNT Terminal Server

So the answer to my above questions might give us a better idea though there could be other questions ..

Have you tried to update your outlook going to http://officeupdate.microsoft.com ?
