When trying to open control center for DB2 UDB on an NT platform, I get the following message :

javaw.exe - Entry Point Not Found
The procedure entry point sqlochgfileptr2 could not be located in the dynamic link library DB2SYS.dll

Followed by the message :

[IBM][JDBC Driver] CLI0622E  Error accessing JDBC administration service extensions.

and control center does not start up. I have tried re-installing DB2 several times to no avail.

what version have you tried to re-install?
in order to re install, you must un-install first, in nt this means  add/remove programs, choose remove, make sure it is a 'clean' un-install, if you have done it this way, was the un-install successful? If you had un successful uninstall, registry problems will create the type of error you have.
If successful, make sure you manually remove all the db2 sub directories under sqllib or wherever you installed db2.
Re boot the machine, make sure you have no services listed for db2 or admin instance in services panel of nt.
You are now ready to re install db2.

If this is the procedure you followed and you are getting the above message, it most likely means you tried to install a fixpack which did not complete successfully, or your original install had a fixpack applied and you didnt uninstall completely.

You can always try dropping the das instance and re creating it, have you verified that the das instance is up and running in services panel of nt, most likely the service isnt started.
