Online data entry for applicant

I need an online information system that does the following

A person logs on and enters contact information for an applicant
The a series of a LOT of multiple choice questions have to be filled in and tied to the applicant information

Is there a packaged template or coldfusion application somewhere?
I would prefer to do this in Dreamweaver and coldfusion with either an Access or SQL Server database

I can use the "Employee Information" template that comes with Macromedia.  I already have that in place.
