The icon appearing next to records created in a custom form is the notes icon instead of the contacts icon

I have created a custom contact form in Outlook XP.

For all records created using the custom form I have the notes icon instead of the contacts icon.

Can anyone help me to change the icon.

Go back and edit your custom form.
On the PROPERTIES tab, you can change the two icons associated with the form.
