COM Add-Ins in Visual C++ 6.0 ???

I want to add own Menu Item next to Menu of MSWORD, so I must deal with the COM Add-Ins.
Anybody knows how to make a COM Add-Ins in MFC??? I can make it with VB, but dunno how to do with MFC.
If you can give me a tutorial, that would be so great.

you can use ATL or MFC

you might want to check this 
this one explains for outlook but the concept is the same
