how can i add a selectbox over an image ?

how can i add a selectbox over an imge just like adding a image map over an exicting image.
Check this page www.vanwest.com, i want to move the selectbox found at the bottom of the page and move it at the top beside the contactus link.

You can make the image as the table background and just create a form over it. Like this:
