Win2k network file lock doesn't always work.

I have a 10 member 2000 AD Domain, MS DNS and DHCP running on DC.

All clients are ghost-imaged Windows 2000 Pro workstations in perfect operation for 3+ years running Office XP Pro 2002.

I have a single workstation user who consistantly is having word files corrupted when the network word doc he is working on is opened by his secretary without any "File in Use" prompt to "open a read-only copy" appearing.  She makes changes, saves the doc and blammo, the doc is hosed as they both have it open.  This happens sporadically.  Does anyone know where I would start looking?

Originally I thought it was a Word issue and updated all clients workstations (both OS and Office) to latest patches.  This did not help.
