creating forms in a blog

we have a blog which uses wrodPress and want are visitors to be able to submit reviews of restaraunts.  They would have to be reviewed by the moderator before posting and we don't want them to submit reviews via comments.  
Is there some way to create a form with all the relevant fields which users could fill out and submit to us for review?

How do you intend to proces the form and store the information? a form is just client presentation, it needs to be processed on the server.

Unless you have a very simple suggestion of how to create some sort of database I'm thinking that all info collected from the form will go to an email address

Well you can use mailto, but it does not format very nicely, and has reliability issues because it uses whatever email client is installed on the client:

<form action="mailto:youremailaddress@yoursite.com">

No there is no magic bullet... just sweat and tears.  The more you want to do with limited technology; the more complex it becomes.  Any additional functionality extracts a price. The normal way that websites handle information transfers is with server side programming, so the bits and pieces available client side are not really designed for serious web app work.
