Adding M.D. to a contact

I have aclient that wants to add M.D. to the suffix of anybody that is one, to their contact info in Outlook XP.
The problem is that they want it to it to file as:
lastname, firstname suffix

I can type in the M.D. in the file as area but that defeats the purpose of having the suffix.

Is there a way to change the file as settings?

In the new contact window, click the "full name" box, which will give you a separate field for a suffix.

Yes I know, But how do you make the suffix show up in the File As Title.  So the MD is in the name when viewing all contacts?

Neither answer is a solution.  

The solution is to file as lastname, firstname and then type in the M.D.  You also have to put M.D. in the suffix.

Close this when you come across it.
