Hi,

before you start developing your web application forms and pages, you should think off of 2 important things for your web application.

1st. Backend database to use. This is important and crucial not only on how many data you can handle via the database (like Access, SQL Server, MySQL, Oracle, etc), but it's on how to create a normalise entities (tables) to storing your data. More advanced RDBMS means you can handling better for your data at server side.

2nd. Server side scripting language. This is necessary, since you're creating a form for data submission. We will then pick a Server side scripting language to use to connect with the database. The Server side scripting language mentioned here is as like ASP, JSP, ColdFusion, ASP.NET, php, etc.

So, if you're hosting your site on Windows, i will suggest you using combination of Access + ASP (for simplest implementation), SQL Server + ASP, Access + ASP.NET or SQL Server + ASP.NET, etc.

It's all depends on how you gonna host your site and how much effects you want to contribute to your web application.

Let us know if you need further information.
