Public Folder Forms - How Exactly?

I have a requirement to be able to post a change request document to a public folder then have an e-mail sent to a group for approval, then once approval is given notifcation e-mail of the approval should be sent either to the same group or to the request originator.
Never having dealt with forms before its a tad confusing to understand how or where to start?
Running Exchange 2003 and Outlook 2003.

    This is quite a large topic to answer.  I'd recommend you get a book on programming Outlook.  Something like "Programming Microsoft Outlook and Microsoft Exchange 2003" or "Building Applications with Microsoft Outlook Version 2002" (I haven't seen the same title for 2003).
