Requesting general info on creating FORMS web pages

Greetings wise wizards of EE:

I am a somewhat fledgling web designer, that has mastered the basics of Dreamweaver, Fireworks & Flash, to create some visually attractive pages, but limited in interactivity. I am able to create basic pages with standard interactivity, such as links, image maps, e-mail links, rollovers, animation & animated buttons, but I have no idea how to create FORMS, nor do I even know where to begin to look...

...I use the antiquated, but still useful Dreamweaver 4.0, which does a fine job, until I eventually upgrade to Dreamweaver MX (or whatever the current version is). I am a complete newbie with FORMS, and I know that Dreamweaver can create the form page template, but not the intelligence behind them. My questions are:

1. Where should I start to learn about creating FORM pages?
2. What application(s) is used with Dreamweaver to create the intelligence behind FORMS?
3. Any links to help me enter the world of FORMS would be mucho appreciated.

Well..,

Using forms is an everyday process in web development.  Anytime you want to store user information in a database, or gather user feedback on your site, or post a blog you are using a form.

1. Where should I start to learn about creating FORM pages?

2. What application(s) is used with Dreamweaver to create the intelligence behind FORMS?

It depends on what type of server side language you're using.  Dreamweaver has templates and wizards that use several server side languages, (i.e. ASP, PHP, ASP.Net, CFM, JSP), it's up to you to choose one that you feel confortable with.

3. Any links to help me enter the world of FORMS would be mucho appreciated.
