Best program/environment for writing/re-writing/saving employee appraisals?

In the past, our company has used a WORD template for employee performance appraisals.  I need advice on how a Front Page/web form appraisal that I created could be used for multiple employees over several years.  I need to be able to pull up a blank form for each employee in each year, complete the form in one (or more!) sittings (need to save it), and then store and retreive the form.  Thanks for your help!

You could use a database of sorts.  FP will help you dump the information inte a MS Access database.  FP will even create the DB for you from the form you create.  You will need to be a on a Windows server though
