Verifying Adaptec 39160 driver for Windows 2000 - three lights on DDS4 blinking - Veritas job paused

I want to verify that I have the correct driver installed for my Adaptec 39160 dual channel card in my poweredge 2600 Windows 2000 server.  I have attached a LTO Ultrium device to one channel and the dell dds 4 to the other chanel.  (Dell shipped it connected to the perc, or connected to a SCSI controller that didn't show up in Device Manager.)  Veritas still runs nightly backup to the DDS 4 and it ran fine for three nights after install.  Then today, all three lights were blinking on the DDS4 and veritas reported it as offline.  If you scroll to the bottom, you wil see that it seems there was an issue with the adaptec card failing to find the DDS 4.  The current driver is:

Adaptec 3960D
Ultra 160/m PCI SCSI Card
Microsoft 5.0.2195.6717
Microsoft Windows Publisher


Also, after reboot, what are all these errors?

I have the same problem, but when I verify and restart Server the dumb blue screen appear with following error : "IO system verification ERROR on 04mmdat.sys (WDM DRIVER 226)..." and then it dump memory and restart. Please help.

Can you boot into safe mode?  Is it rebooting and rebooting and rebooting?


I can boot up now with safe mode(no more rebooting) , but the DCOM problem still there. Please advise.

In safe mode, disable the tape drive

My problem, I think, was a poorly seated tape -> eject the tapes.

Pull the card and the tape drive out if running the server is more important

Otherwise uninstall and use older drivers for the card.

Did you get it going?  What did you do?  To get the others in this thread to respond, i could re-open this issue.

What parts of your issue were exactly similar to mine and which parts varied some?

Because the server is in testing mode so I am still keep everything there. I still can backup with Veritas Backup Exec 10, and I am thinking of install everything again. I think my problem were exactly the same as yours. Please tell me how you overcome this problem without reinstalling everything.

I removed the tapes and rebooted.  Several of the "errors" were considered normal and the issue was freaky.
Just because the verify blew up doesn't mean anything tragic, sometimes the debuging utilities are awful
especially if the driver isn't written to MS standards.  What is the version of the driver you are using?
Do you have a LTO in there too?  Can you tell me how you built the test server up to the point of it
bombing?  Is it 2000 server or 2003 server?

I have window 2000 Advanced Server running on IBM Xseries 255, Internal Tape Drive Seagate DAT 4 DDS , External HP LTO Ultrium 230. This server is backup server & testing server. Last time we use window backup tool to backup data but it is not so useful so we decided to use Veritas Backup Exec 10. After installing Backup software and do some updating, when restart it have this problem. It is not effected  to Backup job but I am worry it have some problem in future. I have checked and find out that the problem is the conflig between Microsoft Tape Driver and Backup Exec Tape Driver ( and Tape is using Backup Exec Driver), I have changed driver to Microsoft Driver then DCOM problem disappear. Right now I still have a red even viewer : "The driver detected a controller error on \Device\Scsi\nfrd9601".

Ok and what is the controller card you are using?  Does it have its own diagnostics?  Backup Exec likes to use its own drivers but the Adaptec card definately wants the microsoft driver.  What devices are attached to the scsi card, just the two tape drives?  Detach tape drive A from it, reboot and check for the error, then reattach that one, deattach the other one, reboot and check for error.  i was going to, if my problem persisted, put a different scsi card in the computer for each tape drive.

We are using Adaptec AIC -7899 Ultra160 SCSI version 7.0.4100.100, and IBM ServerRAID 4Mx Controller (winnt\system32\drivers\nfrd960.sys).We have two tape drives attached only. Internal Tape attached to Adaptec SCSI, external LTO attached to channel 2 of ServerRAId card. I think the message show the problem with ServerRAID Card.

Yeah RAID cards are only supposed to be used for RAID Hard Drives.  Put the LTO on its own Ultra 160 SCSI card.  If removing the LTO from the server RAID card does not fix your problem with that card, call IBM for a new RAID controller or for their input.  Are there diagnostics for the RAID card you can run?

I don't have any diagnostics for the RAID, only SeverRaid Manager, but this tool doesn't show any problem of RAID. I will try to change to Ultra 160 SCSI card as your suggestion and see what will happend. Thanks.
