If all connections go stale

If all our connections in the connection pool suddenly go "stale", does that point to a a database problem (i.e, the database shuts down) or could the problem still reside on the app server side?

That is a pretty normal occurrence.  Your application will get a
StaleConnectionException and should just retry the code.

What happens is WebSphere does not do keep-alive's, and
the database side times out.  As soon as WebSphere detects
a problem it will re-initialize the pool, and all will be set again.

A good solution is to set you min DB connections to "0" in
your pool.  That way WebSphere will take the pool down
before the DB zaps the connections.
