troubleshooting Question

NMS Formmail Not Working

I am trying to get NMS Formmail to work on my host.  It is driving me crazy.  I thought I had a problem uploading the script but it seems more complicated than that.  Here are some of the variables:

1.  Path to PERL /usr/bin/perl
2.  Path to sendmail /usr/sbin/sendmail
3.  A test form located at:  www.summitre.com/vera.htm

Here is the contents of that script:

If anyone can help me figure this out it would be greatly appreciated!!!!!!!!!!
