sounds crazy to me, but it happened at least twice
I assigned the telnet password and enable password to be the same thing.
[md5 hash= e6DpyUwXIMxAEqi for anybody with a working rt hash decoder.]

after "loosing the password" the first time and did tftp recovery, I made it super simple and taped it
to the device on a postit.
It won't take it again after a couple of months.
is it a product defect? hack attempt?

I have telnet access limited to an internal IP range so hack attempts are somewhat limited to that??

best regards
