Trouble defragmenting NTFS Win2k Server partition

I have a NTFS partition running Win2k Server that I'm trying to defragment.
Every time I try to defrag it, it runs for 1-2 mins then finishes and you can see from the report that it didn't make any changes. It didn't defrag it.
The partition is 4gb (16% free space), total fragmentation is 16% and file fragmentation is 26%.
The cluster size is 512b (checked because of article http://support.microsoft.com/kb/227463)
I've also tried a complete scandisk beforehand or defrag in SafeMode.

Anyway to do this from DOS?
Is the problem caused by NTFS?
Could I access and defrag my NTFS partition booting from another partition (WinME, Fat32) on the same PC?
(If it's possible, I don't know how to acccess the NTFS partition)

Try to free up some more HD space, and then try.
I know that defrag does not work is HD free space is less than 15%. May be, 16% too is not sufficient.
