OK Guys, I know you are all groaning as you've seen this one so many times before.....
Situation:
----------
New Hard Drive in old PC.
Build from scratch.
Windows 2000 Professional.
All installations done.
All critical updates & software realeases done.
After hassle, found drivers for graphics card. Done.

Just one thing left. THE MODEM
I've never seen a PC like this one, it has a separate small socket at the bottom of the mother board - after the usual PCI slots - with a modem card plugged into it.
This slot is 19 pins + 11 pins unlike a PCI which is 49 + 11
The PC motherboard is I think an AWRDACPI

Whenever this modem is plugged in, it is detected as unknown "PCI Simple Communications Controller"
And resides on PCI bus 0, device 7, function 6

The card has no defining marks that I can find. The only lettering on it is as follows:
IMRS-R
010514906
NC5IM00496E
and on the reverse:
6694v0

Can anyone help with this one?

Cheers.
