Populating text fields with a combo box

I have a drop down combo box which displays a customers last name, from this drop down box i want to populate the rest of the fields such as address,postcode etc from the database using the customers last name.
here is the code i am using so far;

There are a couple ways to do that but the most common would be to use the Javascript "onChange();" method in the combo box so that it will get the new html page with the address fields, etc, prefilled.

The other way is using something called AJAX which does not require you to refresh the page but is more difficult to implement.
