Maybe it's this: Notes seems to check all databases that are present when it is started. That means: all files with the extension .nsf might be added to the workspace. Could it be that there is a directory somewhere with the .nsf extension??

Looks to me like using the ncompact.exe to compact the database resolved the
issue or apparently did, since the error message finally went away. There is a another
archive directory outside of the Lotus Notes directory though that does have
a *.nsf file in it, they use that when they travel to reply offline and save as
a draft then send it when connected to the network. So that could also have been
a issue but they have never had this problem and have done that for many months
like that previously so I'm going to have to say the database compact fixed it.
Thanks!
