You will need adobe acrobat software or other PDF Form creator but my suggestion is Adobe Acrobat.  With that software, you can create form to let user fill it out online and print it out.

Any idea of minimum version with this facility, I have Acrobat 5.
As long as what you have is not Adobe Acrobat Reader 5 but Adobe Acrobat 5 then you should be able to create editable pdf form.

Are you asking how to link a .pdf file to a web site? If so try this :

<a href="http://www.your_domain.com/filename.pdf">Text link</a>

** If there is no Adobe acrobat or acrobat reader installed in your computer then the .pdf file will start download instead of opening into browser.

All you need is upload your pdf document to the web server (or where you host your website) then you create a link as what rsdn said.
