User Controls in winforms project disappear

We have a multi project vb.net winforms solution under Visual Source Safe 2005.
One project was the UserControl library. We had many issues with every usercontrol disappearing on every form so we moved the usercontrols into the project that contains the forms.
Now one form still loses controls. Other forms do not.
Each developer has been losing the list of controls from the "My User Controls" tab of the toolbox. If we open a usercontrol in design view and save it, it reappears in the toolbox.
Does anyone know what is going on?

Did you try adding the item to ToolBox manually, and not depend on the IDE to keep track of "My User Controls".  When you do it that way, you can turn off this setting:

Tools | Options | Windows Form Designer | AutoToolboxPopulate

When the controls were a separate project, I did add them manually. Now that they are in the same assembly as the forms, I don't know if that's possible.

You can add any control from any assembly to the ToolBox.
