Yes, BACKUP.  Any time you make a major change like this, you WANT to backup the system. 99 people out of 100 can tell you they didn't have a problem.  Do you really want to join the 1% that did because you didn't make a backup first?   Make sure you make a System State backup.

Though lots of people have had some issues, they seem to be few and far between. Personally, my only caution is if you have certain Dell or HP servers, there are a couple of specific problems that require vendor updates before installing SP1 see the following, extracted from:
The above is the SBS site but the issue is for all SP1 installs. In the event any of your DC's are SBS's make sure you read that entire article. SBS requires 5 steps to install SP1 rather that the 1 step for Server 2003 standard/enterprise etc.

Vendor specific issues with Windows 2003 Service Pack 1
Primary Dell page for 2003 SP1:
General Dell pre-install instructions for SP1:
Stop error KB article from Dell –includes a link to the Dell Registry Update tool:
And from Microsoft TechNet post about Dell’s OpenManage support for SP1: Dell OpenManage support for Windows Server 2003 SP1. In order to support the new enhancements and features in Windows Server 2003 SP1, Dell plans to release Dell OpenManage version 4.4, including Dell Server Assistant version 8.6, to fully support SP1. For current customers who are running Dell OpenManage version 4.3 or earlier, go to the Dell Web site for Dell OpenManage and Service Pack 1 upgrade and deployment information.

For HP Servers please refer to: HP Support. Software and Drivers - download ProLiant Support Pack for Microsoft Windows Server 2003, 7.30 A:
