Looking for software to monitor physical Disk I/O

I am looking for a utility that will have a graph for each disk/partition that is similar to the Windows "Task Manager" performance tab only instead of the graph showing CPU Usage history, it would show I/O history in a histogram.

Anyone ever seen anything like that?

paste  this in the "run" at the start menu
this will open performence monitor

(if u working with win XP or server ...)

note that you can add (pluse sign) any counter that you wish.

you have there a button with (+) sign so with this you can add
counters to the graph ...
