Help!!!! Need form results to go to specific e-mail addresses dependent upon clinical trial

I have a PHP form that submits applications for clinical trials here: http://www.stormontvail.org/cancer_ctr/  However, each trial needs to get routed to a different e-mail address depending upon the type of cancer the clinical trial is for.  What's the easiest way to accomplish this?

Have a database table
id, type of cancer, email

depending on the type_of_cancer query the database for the email.

I was afraid of that.  Can you think of any examples to get me started in creating this?

If there are only fixed number of trials, you can even hardcode it
like the following url needs

to be modified to

have a hidden input variable to grab the email address just as you do for trialname.
