Customising the General tab of a journal form

I would like to use a custom form for Outlook 2003's Journal. It is simple enough to create and edit additional 'tabs' to the form, but I simply want to add a check box to the 'General' tab. Can this be done?

Sorry, this isn't possible.  The first page of the Journal, Appointment, and Contact forms cannot be modified.
