Need web code sample / site for product download

Hello -

I'm looking to offer a product on my main web site. I need a validation form, requesting name, company name, telephone number, and email address. Once the user enters the data, they click a check box to accept the terms listed in a text box just below the other fields.

From that information, they click a submit button and I get the contents they typed and they are redirected to a page that says 'Click here to download'. They are followed with a prompt  'Open...SaveAs....or Cancel' dialog that pops up.

Well you want to do web programming. Now you can use quite a bunch of things for that.
PHP+
ASP+
JSP+

etc.
Or maybe just starting with HTML forms, see e.g
