Element 'html' is missing required attribute 'xmlns'.

I noticed I got this error all the times in all pages at the <html> tag:

Validation (XHTML 1.0 Transitional): Element 'html' is missing required attribute 'xmlns'.

Any idea what it is?  And, how to get rid of it?
