I have multiple sites running on IIS 6.0 and for some reason the .vb file type is served on some sites and isn't on others.

I have .vb added as test/vbscript in Mime types.

Server Error in '/' Application.

This type of page is not served.
Description: The type of page you have requested is not served because it has been explicitly forbidden. The extension '.vb' may be incorrect. Please review the URL below and make sure that it is spelled correctly.

Where else can I check?

Yes you have to define mime type of .vb but ensure that .vb extesion file not run if you not added in the application extension the error  Server Error in '/' Application. means you didn't add application extension.

Solution :

1. HAe you install ASP.NET 2 and allowed website extenson ASP.NET 2
Note: if you are using ASP.net 1 then install version 2
2.Have you add application extension check through the following
IIS>website>select website>properties>Home directory>configuration>application extension > Add >
extension .vb and file path as below
