Take a look at the fonts used in the chart.
Some are available to both printer and screen, some only to screen, some are scalable. Assuming the creation with Word and exporting to PDF not made by Adobe's Distiller, sounds the font used at creation time isn't available at print time.
MS Word shows different icons for each type. To be portable, consider the use of very common fonts as Arial and Times New Roman (you'll notice a TT icon in the font selection list) and avoid rare fonts.

Adobe Acrobat's PDF files are supposed to support printing as viewed at screen, so the problem should be at exporting time. If you watch the chart correctly in the screen when in Acrobat Reader, take a look at printer settings. If there are colors, the conversion to printing can be the cause.

Well I got the org chart which is created from powerpoint. After it is converted to pdf, I check under the document properties it did not list any of the fonts type that means the whole powerpoint is convert to PDF as a image. If I print from the ppt it look fine the problem comes when printing from the pdf, whereby the org chart boxes will be printed as black. I have try update the reader the problem still prevalent. Any people have same problem as me and found solution for it.

I have found the solution to my problem. At the print menu of acrobat reader/writer, click on the advanced button to access the advanced printing option. make sure the Print As Image checkbox is checked. Then the pdf will print fine with no black patches.
