Outlook form will not open on a specific computer

I have created an outlook form and saved it as an OFT in a shared public folder.
I can open this file from many computers (over 20) but on one computer it will not open.
When I click the OFT to open the form a new message opens. The form data is not dispalyed at all.
I have logged onto the computer with the problem as my username and password (2000 Domain environment) configured my email and i can't open the oft from this computer either.
I have checked to see if any outstanding patches are available and there is none.
The poroblem computer has outlook version 2003 (11.8010.6568) SP2 which is the same version as mine which opens the oft file correctly.
If i save the form as a MSG then the document can be opened by the problem computer.
The problem computer seems to open other organisational forms but not this one.
Any suggestions

Are you saying that a new message form opens, not the custom form?  Or are you saying that the custom form opens, but parts of it are missing?

I am saying that a new message form opens, not the custom form.
only for this one form all others seem to work great and this form on other computers also works great

Sorry about the long delay in getting back to you. I have just migrated to exchange 2003 and upgraded several servers and it had my complete attention.
