Can someone please give me some code for putting the icon that is associated with an unknown file extension into an imagelist?

I have looked everywhere for it.

The project here will show you how to do everything and anything you ever wanted to know about how to make a "windows explorer" type interface...including how to extract default icons (drives, folders, etc.) and icons associated with a given extension.

I assume that shell.dll (index = 0) contains this icon?

You are correct.  It is at %SystemRoot%\system32\SHELL32.dll for Windows XP.
