Html Help Workshop - Getting Started

Ok, two parts to this one.  First off, can someone give me the basic steps for creating a help file using HTML Help Workshop.  I've seen all the pieces, but I'm just not sure which to start first.  As an example, if I were creating a windows app in VB.net, it'd be:

- Start a new project
- Add controls to the form
- Add code to the form and controls
- Add additional forms as needed
- Build the project

As you can see, I don't need detail, which I'm sure I can find in the help files once I get into it.  I just need the steps so I know where to start first.  

The second part to this question is related to connecting it to the VB.net App.  The most important piece is that when a person hits the help button, it goes to the appropriate page.  How do I connect the code to the help file?

It looks like your article is exactly what I needed.  One issue though I can't seem to get.  When I first create the project, everything goes very well.  But if I have to quit the application for each reason, when I come back to open the project, I can never get the nice window to reopen with the Contents and Index tab.  What do I have to do to get back to that screen?
