what are some reasons ActiveX would not load in IE6 and IE7 browsers ?

it is used like this:
<html><body>
<script src=script.js></script>
</body></html>
and script.js that contains document.write("<OBJECT ....></OBJECT>");

ActiveX cab is not a problem, it used to work ok

have you tried embedding the object tag info directly in the page rather than using the script to see if that makes a difference?
Is your classid correct?
Is your path to the active x object correct? Check the upper/lowercase

When you say it does not load, what happens?

yes, classid is correct, path to cab is correct.
here is a demo http://www.kupid.com/chat2.php

some webpage said following IE options need to be enabled:
- Script ActiveX controls marked safe for scripting
- Download Signed ActiveX
- Enable ActiveX controls and plugins
- Active scripting

are there some other need-to-be-enabled IE7 / IE6 options ?

that example works fine for me on IE6, try resetting IE to it's defaults

Have you had any luck?
Also try comparing the ones that work with the ones that don't work, to see if they all have all the latest MS updates?
