Where can I find info about how to read this file.
Below is the sample of hdmp file.

What each column means?  What is the number after the application name? ( What is  +0x117f41? ).

I searched much, but came up with little. Here is a bit of info that may be useful:

I'm guessing that it's an offset memory address, offset from the beginning of the program's address space
