I currently pass information to Outlook, is it possible to be notified if a diary entry is changed in Outlook for it to be reflected within my program.

If by "diary entry" you mean a journal item, then yes.  You can detect a change in real time and you could detect a change by checking the LastModificationTime of each item in the journal and comparing it to the time your program last checked.  Unless Outlook is going to be left open and running constantly, then I recommend the second approach.
