How to create a FP contact page that can be emailed to me?

I am using FP03; I am creating a website and I have a link that points to a "request for quote" page.  I want to create a contact database that can be emailed to me and then redirect to a thank you page.  I tried using the Webbot but it points to ../_derived/nortbots.htm from my website and I get the 404 error that the page cannot be displayed.  I don't mind using this but I do not where this file is.  It must be hidden some place.

hey - do you want a database or do you want the contents of the form emailed to you?
do you have your own server or are you using a web host?

I want the contents of the form emailed to me.  I am using 1and1.com to host my website.  Thank you in advance for your help.

I prefer the MS windows hosting accounts when using FrontPAge
although Linux Hosting accounts can install the front page server extensions (FPSE) - they are problematic at times
Make sure you have installed front page server extensions

once they are activated
create a form on page
on the menu click Insert...Form...Form

you will see a marquee (dotted outline box) with the cursor next to a submit and reset button pair
hit enter a couple of time to make space in the form
add some textboxes and other controls to collect your data (click Help and Search for Forms)
once you have your form completed
rt. click inside of the form and select Form...Properties
click the Send to option...add your email address that will receive the form contents
and VIOLA!
