Offline Addres List generation fails

We run an Exchange 2003 mail server, and have recently been recieving Event ID: 9360, which is immediately preceeded by Event ID: 9341.  Full diagnostics logging during a rebuild of the offline address list adds Event ID: 9109.  We have been recieving this error for a while now, and as all of our clients work in cached mode, we need to correct it so that they will start to recieve address list updates.  We are aware of the registry hack to force exchange to create a full ofline address list, but would like to track down the root cause in order to prevent this in the future.

Event ID numbers on their own are useless. Post the full event ID entry and it might become more clear.
Ensure that there is a replica set on each of the OAB system folders in ESM. You have to have at least one replica listed on each folder.

Anything else changed on the server recently? Change in domain controllers? New Exchange server introduced, old one removed etc?

Event Type:      Error
Event Source:      MSExchangeSA
Event Category:      OAL Generator
Event ID:      9360
Date:            7/6/2006
Time:            2:21:13 PM
User:            N/A
Computer:      APMEXCH
Description:
OALGen encountered an error while generating the changes.oab file for version 2 and 3 differential downloads of address list '\Global Address List'.  The offline address list has not been updated so clients will not be able to download the current set of changes.  Check other logged events to find the cause of this error.
If the cause of the problem was intentional or cannot be resolved, OALGen can be forced to post a full offline address list by creating the DWORD registry key 'HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\MSExchangeSA\Parameters\OAL post full if diff fails' and setting it to 1 on this server.  When OALGen next generates the offline address list, clients will perform a full OAB download.  After that time, the registry key should be removed to prevent further full downloads.

What is this:

'/o=APM_ORG1/ou=APM_SITE1/cn=Departments/cn=Office Manager'

Have you changed something in relation to that?
Moved Exchange servers in to that, removed it? Something else?

Will my email signature work in Office 365?

You've built an email signature using raw HTML code in Office 365, but you can't review how it looks with Transport Rules. So you have to test it over and over again before it can be used. Isn't this a bit of a waste of your time? Wouldn't a WYSIWYG editor make it a lot easier?

i realize that the the problem with the OABgen is related to

'/o=APM_ORG1/ou=APM_SITE1/cn=Departments/cn=Office Manager'

however, no changes were made regarding that OU in Active Directory.  is there a tool i could use to futher research what is wrong with it?

What is in that OU? Anything Exchange related? Objects, groups users etc?

that OU contains 2 computer accounts, 1 security group (not mail-enabled), and 1 mail enabled user.  those four items have been in that OU for a long time, without any changes being made.  could there have been some corruption to the user account?

The user account would be the first place I would look.
A quick test would be to move it in to another OU and see if the error "follows".

i tried that, and then rebuilt the offline address list, but the error stayed the same, verbatim.  but thanks for the idea.

the only other thing i can think of at this point would be to relocate everything in that OU, delete it, then rebuild the address book to see what happens.  not sure that is really a good idea, though.  suggestions?

If the error stayed the same, then removing the OU is probably something that you need to consider doing - purely to rule out that as being the cause of the error. If the system then coughs over something else then you deal with that.

Work with the obvious - do not go looking for more in depth issues. There might be a simple problem with that OU that removing it and recreating fixes. The fact that it is appearing in the event viewer isn't good, so you need to stop that.
