Outputing Pdf

Hi all

What im wanting to do is take informaton from mysql db and a picture (form)im going to scan into the pc i would like then to be able to download this completed form from my server then print it out

how can i start to do this

so lets say i have a few drop down boxes populated with some db information

i select from these boxes what i wish to be outputed in pdf with the backing picture
i then click a button and download the information selected with the backing picture in pfd

how would i go about this

i can populate information into dropdown with php
i can talk to dbs
create forms
validate forms

just after a point in the right direction please
