Terminal Services in App Mode

Can someone provide me with a link for step by step instructions for Installing and Configuring a terminal server in Application mode?

if you are using windows 2003 the when you install terminal services from add/remove windows compnents in control panel, it automatically would install terminal services in application mode (more than 2 simult cons)
in which case you also need to install a terminal server licensing server.
the licensing type could be either device cals or user cals.
the license server has to be activated and licenses installed.
contact MSClearing house to get the Lserver activated and install cals (these cals have to be purchased in addition to windows cals)
if temp cals are used (no cals installed) they are valid for 120 days, and then they would not allow any client to connect.
