Fill PDF template with data from sql database

I have a PDF form with fields, is there anyway to fill the fields with data from SQL database? The data in SQL database is submitted through vb.net web form. Thanks for help.

There a couple things out there you can use, the best one I have found is ITextSharp.  It is a free control which allows this type of control.

You can find it at:

You will need to do a little research on how to work with the PDF forms because I personally haven't touched that part, but I do know it works.

Thanks for your help. I download itextsharp but don't know how to start.
To start out, put the dll into your "bin" folder.  Then add a reference to it through Visual Studio.  Inside your code:

Imports iTextSharp.text.pdf
Imports iTextSharp.text


Page_Load(...)
{
  PdfReader reader = new PdfReader("pdfdocument.pdf");
}

This is some quick stuff, I never used it with VB.net, but read through the forums and do some research on google.  I do that most of the time myself.
