Greetings, openjohn !

Change folder icon size from Accessibility options. This tutorial is for Outlook 2002, but will work for 2003.
http://www.microsoft.com/enable/training/outlook2002/default.aspx

If no joy, go to Control Panel > Accessiblity and change the font size.

If no joy, you need to change font size of your Display Properties.  Right click on an empty area of Desktop and select Properties > Appearance. Change item to icon and font size to a large size. Save the Scheme.

Best wishes!
