Is there an easy way to make the following case insensitive

You could use:

        If r.ToLower.StartsWith(prefixText.ToLower) Then

        End If

In VB 2005 you can do it this way
 
       If r.StartsWith(prefixText, StringComparison.InvariantCultureIgnoreCase) Then
