xserve g4 external scsi controller

Hi;

Is there a way to verify if my SCSI controller is working?  Some hardware test?  My attached host is not showing up.  Host test show OK.

System:
OS X 10.3.9
2 GB RAM
External
SCSI: ExpressPCIProUL366

Where Can I get the apple hardware test CD for xserve g4?  I need to burn a new one.
