I am looking for a nice app to make forms. Preferably drag and drop type editing and fairly clean code output. I can hand code them but I have a lot of forms I need to do and not a lot of time to do them. The simpler the operation the better.

Have not used this.  But, it is free which usually means simple.

You said you have already GoLive CS, I would do it with that.

I have dreamweaver and GoLive but they seem to make the code pretty messy. I want to be able to open it and tweak it once I get a the required fields on it.

Web Weaver looks pretty nice for what I need it to do. Also it helps that it is cheap.
