Signup form

I want to create a signup form that saves all form values to MySQL database

the form values are:
- username
- password
- fname
- lname
- state
- city
- address
- country
- zipcode
- referrer
- tim
- tia
- balance
- paidbalance
- adblast
----------------
and how can a member login with his or her username and password and be able to update his information and how can I update the member's information via a form that is accessible to only me.
