My return email address is wrong; delivery failure if someone replies to my email

When people reply to my emails they get a delivery failure. My return address looks like this:
firstname.lastname@example.org Sent by: email@example.com
or like this:
From: firstname.lastname@example.org [mailto:email@example.com] On Behalf Of firstname.lastname@example.org
This started to happen some time after July of this years. My return email address should only be email@example.com. I don''t know if the problem is happening within my Lotus Notes Client or somewhere
on my Comcast account set up. This is causing a big problem for me. I did look for solutions on experts-exchange but the only one that I found that was close did not give step by step and I couldn't fix the problem.
The Notes Client is release 6.5.4 dated March 27, 2005.
I am running a windows 2000 operating System.
